Remote Learning 2021: CAD options from a foot scan: CAD positive or Orthotic shell

In CAD design at this stage there are 2 options of what you can design from the foot scan:

1) Design a CAD positive:

This would be the same outcome as the traditional plaster work and result in a modified positive design that can be milled.

A CAD positive is commonly milled from MDF (wood) or foam and used to vaccum press a thermoplastic to form the orthotic shell.
A range of thermoplastics can be used including EVA, polypropylene, carbon and subortholene.

2) Design an Orthotic shell:

This skips the step of the CAD positive and directly designs the orthotic shell.

The orthotic shell can then be manufactured using subtractive or additive manufacturing methods out of a range of materials including EVA, Polypropylene and 3D printing polymers.

Behind the Design Series: Sulcus length Cover:

In behind the design we look a one aspect of the orthotic design and discuss its selection.

This client needed a custom orthotic to fit a range of casual footwear, with different shapes and styles.

A sulcus length cover was selected from length options in the Top Cover section of the DOLA detailed prescription form.

Sulcus length cover features:
- For shoes with or without full length removable insoles
-replaces part of the shoe insole space
-protects the plantar foot including Plantar Metatarsal Area to the sulcus
-protects forefoot modifications including domes, met bars, forefoot extensions
-takes up reduced space in the shoe
-increased ability to change between shoes

Orthotip

The orthotic length or orthotic cover length is a key prescription variable, as a successful choice will allow the patient to wear the device in their favourite footwear increasing the likelihood of patient compliance.

Before an orthotic is prescribed a clear plan about footwear selection and even changing the patients footwear choice should have been made.

3 common cover / orthotic lengths are:

Full length
Sulcus length
3/4 or shell length

Each style has positives and potential negatives about its choice that should be considered.

Remote Learning 2021: Scan preparation for Computer Aided/Automated Design

This traditionally involves the transformation of the negative into a positive foot model, as discussed last week.

In CAD this step involves importing the scan into your choice of software and preparing it for design.

An STL file type will generally be able to be universally imported. Other file types may need decrypting before use.

Why is there encryption?
-between different scanning systems
-between different labs
-security

Steps in preparing the scan for design:

-Remove unwanted artifacts
-Trim the scan
-Assess the scan
-Check calibration

If the scan is unfit for purpose, contact the clinician and discuss options.

Behind the Design Series: Medial Heel Skive (Part 2) Angle of Skive

In behind the design we look a one aspect of the orthotic design and discuss its selection.

This client wanted to modify the angle of heel skive from the lab standard 15 degrees.

They used the Instructions section of the detailed form to request 10 degree angle resulting in a less aggressive skive.

The medial heel skive is one of the most popular orthotic prescription variables.

Intrinsic (in-shell) modification is used to create a varus / medial heel wedge.

Choose the:
Depth of skive (mm) (discussed last week)
Angle of skive (degrees) Lab standard = 15 degrees

Orthotip:

The angle of heel skive can be modified to achieve your goals.

Lower angle: more gradual angle, reduced force over a larger area

Higher angle: steeper angle, increased force over a smaller area

The depth and angle of the skive work together and can be modified to achieve your treatment goals.

Uses:
When needing Medial / Supinatory orthotic reaction force.
